We are recruiting a Care Coordinator to support the effective day-to-day delivery of an established domiciliary care service in Bristol. This is a hands-on coordination role acting as a central point of contact between service users, families, care staff, and healthcare professionals.
The Role
- Coordinate and manage care packages ensuring timely, person-centred delivery
- Prepare, manage, and update daily and weekly staff rotas
- Allocate care calls based on staff availability, skills, and location
- Arrange cover for sickness, annual leave, and unplanned absences
- Monitor missed visits, late arrivals, and changes in service user needs
- Maintain accurate care records, care plans, risk assessments, and MAR charts
- Conduct audits, compliance checks, and support CQC inspections
- Support staff supervisions and spot checks
- Liaise with social workers, GPs, district nurses, and local authority professionals
- Escalate safeguarding concerns, risks, and compliance gaps promptly
- Minimum 2 years experience within domiciliary care coordination — essential
- Strong rota management and care allocation experience
- Experience maintaining care records and completing compliance audits
- Knowledge of CQC requirements and safeguarding responsibilities
- Confident using Microsoft Office and electronic care management systems
- Full UK driving licence and access to a vehicle with business insurance — essential
- NVQ Level 3 in Health and Social Care (desirable)
